To update details like your name, address, phone numbers or email address:
- in your NHS account (through the NHS website or NHS App)
- in your SystmOnline account
- fill out a change your personal details form
- phone us on 01992 450040 or visit the surgery
Change of address
If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery. We are unable to keep anyone registered who moves outside of this boundary.
Changing your name
You will need to show us proof that you have changed your name. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
