Self referrals
There are a number of services locally that you are able to make self referrals to.
- Anxiety
- Carers
- Drugs and alcohol
- Exercise
- Maternity
- Sexual health
- Smoking cessation
- Young carers
- Youth counselling
- Weight management
- Abdominal Aortic Aneurysm Screening (Men 65+)
GP referrals
To request a GP referral:
- fill out a referral request form
- use the Accurx service
Provide as much information as you can as this will help us to process your request Usually a GP appointment will be required before a referral can be made.
Check Hospital waiting times
To check waiting times at hospitals by area and department, you can use the My planned care website. This site is updated weekly with current information provided by hospital trusts.
Tracking a referral
To track an existing referral:
- visit the NHS e-Referral Service
- use the Accurx service
